2009 Return for Household Employers - Unemployment Insurance Contributions (2009 Form UI-HAR - Retroactive) Annual contribution and wage reports, 2009 Form UI-HAR, must be filed by each household employer subject to the Illinois Unemployment Insurance Act who should have and did not file a 2009 report. The employer needs to contact the Illinois Department of Revenue regarding past due withholding taxes.
