Administered:Classes start in September and end in March. Exam given at each class.
Exam Fee:$150.00 materials fee per class
Passing Criteria:Grade of 80% or higher plus satisfactory practicum performance

Citizenship:No requirement
Other:Certificate renewable on three year basis by attending Illinois Department of Public Health (IDPH) sponsored workshop.
