UI-HA Report for Household Employers (Tax) Form & Instructions 2016
IDES form used to report wages of household employees (Illinois).
State Experience Factor & Employers' UI Contribution (Tax) Rates
For 2017: the minimum UI tax rate is 0.550% and the maximum is 7.350%. Only the first $12,960 of an employee's wages are taxable.

The 2017 State Experience Factor is 106%. The State Experience Factor for 2016 was 112%.

Unemployment Insurance Notice (Poster)
This poster fulfills all posting requirements for the Illinois Department of Employment Security. Employers are required to post this notice in a conspicuous place for all employees.
Report to Determine Liability Under the UI Act (Form UI-1)
This IDES form must be filed within 30 days by any employing unit that commences business in Illinois, whether by purchase of a business already being operated, by starting a new business, or otherwise.
Employers Claim for Adjustment or Refund (Form UI-28)
This IDES Revenue form is used by an employer to file a claim for adjustment/refund not later than three years after the date upon which any contributions, interest or penalties were paid erroneously.
Notice of Change (Form UI-50A)
Employers use this form to report name, address, telephone, or other miscellaneous changes to the business. It is also used to close a business UI tax account.
Employers Contribution and Wage Report (Form UI-3/40, Quarterly)
For employers filing a paper-style report, quarterly contribution and wage reports are made using Form Ul-3/40. Note: TaxNet is the preferred method for filing both monthly and quarterly reports.
Voluntary Leave Questionnaire - Employers (Form ADJ002FE)
An employer form used to collect information about whether an individual who quits his/her job did so without good cause attributable to his/her employer.
Able and Available Questionnaire for Employers (Form ADJ017FE)
IDES uses this form to determine whether an unemployed individual able to work, available for work and actively seeking work. The information provided will be used for the purpose of determining a claimant's eligibility for benefits.
